ABSTRACT

Chirabilvadi Yoga is a combination of four drugs Chirabilva, Arjuna, Jyotishmati and Kakanasa in powder form, each of these ingredients has solid base of previous experimental studies for their individual analgesic activity. Bhavana (trituration) is an important Samskara (process) mentioned in Ayurvedic classics by which drug may be made very potent to produce a positive results. Till date no data available on this combination Bhavita Chirabilvadi Yoga triturated seven times with Chirabilva Patra Swarasa and made dried powder. Present study is to screen the differences in pharmacognostical and pharmaceutical profile of the Chirabilvadi Yoga before and after Bhavana. After Bhavana significant changes were found in the pharmacognostical parameters like deformated rosette crystals and cluster crystal of Arjuna and broken stone cells of Arjuna, which are rarely found before Bhavana of the drug and also evaluate the differences in the pharmaceutical profile before and after Bhavana like loss on drying, ash value, acid insoluble ash, water soluble extract, methanol soluble extract, pH and HPTLC.

ABSTRACT

Background: Prescription auditing is an important tool to assess and evaluate the drug utilization pattern and rational use of medicines. A successful prescription audit is crucial for health care workers, patients, and the community to ensure that their patients receive the best possible treatment. The purpose of drug audit is to improve patients care and to avoid potential fatal errors. Aim and Objectives: This study has been conducted to evaluate and analyze existing prescription errors in tertiary care hospital of Rajasthan and their magnitude. Materials and Methods: A retrospective observational study was carried out in Jhalawar Medical College, a tertiary care hospital in Rajasthan. Around 1000 prescriptions were collected randomly from medical college pharmacies. Outpatient prescriptions from all the major clinical departments available at hospital pharmacies were analyzed using the WHO prescribing indicators. Information regarding the patient, doctor, drug, and legibility of the prescription were obtained. Results: In our study, we found that most common age group mentioning in prescriptions were 18� years (55.7%), most common gender are males (60.2%). Generic names were prescribed in all prescriptions (100%). Patient information was mentioned in 73.2% prescriptions. Warning signs were not mentioned in any prescription whereas 10.2% prescription mentioned medicines name in capital letter. Average number of medicines per prescription was 4.1. Prescription with drugs prescribed from essential drug list was 88%. Conclusions: In our study, we found many insufficiencies, lack of clarity and important instructions in different parts of prescriptions. There is immediate requirement of improvement in prescribing habits of doctors. Proper steps such as workshops, practical training, and continuing medical education should be needed to guide the physicians to promote rational prescription.

ABSTRACT

Objectives: Neuroborreliosis is generally known to be a disease confined to the Western part of the globe. It is not commonly encountered in this part of the world. Interestingly, we recently came across a series of cases of Lyme’s disease with a plethora of neurological presentations. Most of the cases were a diagnostic dilemma, with poor response to immunotherapy and on subsequent evaluation all were found to have positive Borrelia antibodies. Materials and methods: Eight cases were selected from the tertiary care hospital in North western India. Patients were suspected to have Neuroborreliosis whose neurological presentations were atypical for other classical neurological disorders, who had a progressive or relapsing clinical course and had responded poorly to the initial treatment given for the previous neurological diagnosis. Skin lesions were present in some cases. The patients underwent a detailed clinical assessment which comprised of an elaborate history including history of travel, any insect bite or skin rashes along with a complete systemic and neurological examination. All the required blood investigations, Magnetic Resonance Imaging (MRI) Brain, Computer Tomography Angiography (CT), Nerve conduction study (NCS) and Electromyographic (EMG) studies and Cerebrospinal fluid (CSF) studies were done as indicated in each case. Borrelia antibody titre was done in all the patients using immunoblot technique. Results: Among the 8 patients, 6 were male and 2 were females. The age group was between 25-70 years. The clinical presentation was acute, subacute or chronic. One patient gave a clear history of tick bite. Two patients had skin lesions and one had the pathognomic "eschar". All the suspected 8 patients had either IgG or IgM or both IgG and IgM Borrelia antibodies positive. Almost all the patients had previously received either steroids or intravenous immunoglobulins, but had not adequately responded to immunotherapy. These patients were given a trial of injectable Ceftriaxone and oral Doxycycline. Most of them either showed partial or complete clinical improvement. Conclusion: Lyme’s disease, a common disease of the west does exist in the Indian subcontinent as well. Because of increasing global travel and migration and change in vector habitat the disease seems to have percolated in the non endemic areas too. Proper history of travel or exposure to tick bite is important. We want to emphasize, Neuroborreliosis, a great mimicker may have diverse and varied neurological presentations and has a potential for reversibility with appropriate treatment even after a significant delay in diagnosis.

Background: Evaluation of the pattern of fixed dose combinations (FDCs) prescribing among the practitioners in India. Aims and Objectives: Aims and objectives of the study was to find out prevalence, prescribing pattern and rationality of FDCs in Jhalawar district of Rajasthan. Materials and Methods: Photocopies of one thousand prescriptions of general practitioners, containing 800 FDCs were randomly collected from few big pharmacies of Jhalawar district of Rajasthan and analyzed for the different pattern of prescribing and rationalism. Results were expressed as percentages. After excluding the repetitions, the numbers of FDCs were 218, which were considered for final analysis. Results: Out of 218 FDCs, the most commonly prescribed FDCs were of anti-microbial agents followed by anti-inflammatory drugs and anti-hypertensive drugs. The FDCs were maximum from the department of medicine (30.2%), followed by surgery (20.2%) and Obstetrics (16.9%). Out of 218 FDCs, only 13.3% were rational, and rest of the FDCs was irrational. 94% of all FDCs prescribed with brand names. Conclusions: In our study, most of the prescriptions contains irrational FDCs. Physicians have no knowledge about the essential drugs and FDCs. Therefore, physicians and regulators should be educated regularly by seminars and conferences. Government laws should be made mandatory. Implementation of central drug standard control organizations guidelines on industries for manufacture of FDCs must be made compulsory.

ABSTRACT

Objective:The objective of this paper was (1) to study the prevalence of latent autoimmune diabetes in adult (LADA) in the region of north-eastern Uttar Pradesh, India, based on the positivity for glutamic acid decarboxylase 65 (GAD65) antibodies and (2) to compare the glycemic profile between GAD65-positive and GAD65-negative subjects. Materials and Methods?The subjects were of more than 30 years of age, with either recently diagnosed pre-diabetes/diabetes presenting with the hemoglobin A1c (HbA1c) level of ?5.7% or already diagnosed cases of type 2 diabetes mellitus (T2DM) who had no requirement of insulin therapy for at least 6 months from the time of their diagnosis. All the patients were natives of north-eastern Uttar Pradesh. The GAD65 test was done by the enzyme-linked immunosorbent assay. Further, the glycemic status of GAD-positive and GAD-negative subjects were compared on the basis of fasting blood sugar (FBS), fasting insulin (FI), and homeostatic model assessment for insulin resistance (HOMA-IR). The “unpaired t-test” was used to compare and assess the significance of differences between the glycemic profile of GAD65-positive and GAD65-negative subjects using the GraphPad Prism Scientific Software, San Diego, CA, United States. The p-value of <0.05 was considered to be significant. Results?A total of 77 patients were included in the study, with the age group ranging from 30 to 75 years (47.81?±?12.9 years) with the male–female ratio of 1:2.6. The prevalence of LADA was found to be 51.95%. On comparing GAD65-positive and GAD65-negative groups, a higher value of HbA1c levels and FBS were found in the former, whereas FI and HOMA-IR were found to be higher in the latter. On testing for significance of difference, only FI and HbA1c values were significant (p-value <0.0001). Conclusion?LADA can no longer be considered a rare type of diabetes mellitus, with the present study showing a high prevalence of LADA in this north eastern region of Uttar Pradesh. Identification of adult-onset diabetics accurately as LADA or true T2DM is very crucial for the appropriate treatment, as LADA patients require insulin inevitably and much earlier than true T2DM patients, who can be managed mostly on oral hypoglycemic agents with seldom requirement of insulin

ABSTRACT Objective: To evaluate the effects of commonly used smokeless tobacco forms on oral health at habitual placement sites of smokeless tobacco compared to non-placement sites among the North Indian population. Material and Methods: This cross-sectional study was conducted among 542 individuals using smokeless tobacco recruited from the outpatient wing of the Dental College. Subjects completed a questionnaire and received an oral examination. Periodontal pocket depth, gingival index, plaque index, gingival recession, and oral mucosal changes were assessed. Kendal's Tau test, paired t-test, and chi-square test were carried out to compare different variables among placement and non-placement sites. Results: Most of the subjects were male, reporting an average of 11.26 years of SLT use. Clinical inflammation of gingiva was significantly greater (p=0.01) at placement-sites (1.64 ± 0.53) of SLT in comparison to non-placement-sites (1.40 ± 0.41). The difference in the GR and PPD at placement and non-placement-sites was also statistically significant with p=0.002 and p=0.001, respectively. Clinically, the majority of subjects had mucosal changes at the placement sites, and a statistically significant association (p=0.034) was observed between the duration of the use of smokeless tobacco and the mucosal changes. Conclusion: Smokeless tobacco use predisposes to increased risk of periodontal diseases and oral mucosal changes at the placement sites in an individual due to the local irritant effect.

ABSTRACT

Peritoneo-cutaneous fistula is usually a rare but known entity. Such a case has not been reported after cesarean sections. A 26-year female presented to our clinic in May 2019 with persistent sero-purulent discharge from sinus over abdominal skin. The patient had previously undergone a cesarean section in April 2018 for premature rupture of membranes and non-progress of labour. The surgery and puerperal period were uneventful. The patient presented with document high-grade fever in June 2018. In August 2018, a localized painful swelling was noted by the patient herself over the abdomen, contrast-enhanced computed tomography, which suggested 6.3×3.9×3 cm collection in the right adnexal region and a fistulous tract communicating with abdominal skin. For the same she underwent surgery, wide local excision of the fistulous tract around elliptical skin incision was done. The development of delayed peritoneo-cutaneous fistula has been seen as a complication of difficult cholecystectomy.

Gorlin-Goltz syndrome is uncommon multisystemic disease with an autosomal dominant trait, with complete penetrance and variable expressivity, though sporadic cases have been described. We report a case of 18 years old male patient having features of Gorlin Goltz syndrome. Gorlin-Goltz syndrome is characterized by multiple basal cell nevi or carcinomas, odontogenic keratocysts, palmar and/or plantar pits, calcification of the falx cerebri, and is associated with internal malignancies. It is important to know the major and minor criteria for the diagnosis and early preventive treatment of this syndrome.

Introduction: Inspection of the bone marrow is considered oneof the most valuable diagnostic tool to evaluate hematologicmalignancies. This study compares all three techniques ofbone marrow aspiration (BMA), bone marrow imprint (BMI)and bone marrow biopsy (BMB) in morphological diagnosisof hematological malignancies.Material and methods: The study was conducted on 63selected cases of various hematological malignancies. Onlythose cases in which bone marrow examination was done byusing all the three techniques were included in the study.Results: Out of total 63 cases,53 cases were diagnosed onbone marrow aspirate smears with diagnostic accuracy of84.12%, 60 cases were diagnosed on bone marrow imprintsmears with diagnostic accuracy of 95.23% and all 63 caseswere diagnosed on bone marrow biopsy with diagnosticaccuracy of 100%.Conclusion: It is concluded that bone marrow imprintsare equally useful as bone marrow biopsy in diagnosinghematological malignancies. Imprint cytology shouldtherefore be a standard practice for evaluating bone marrow incases of hematological malignancies.

ABSTRACT

Objective: The presence of specific chemotherapeutic protocols for hairy cell leukemia (HCL) makes it essential to discriminate this entity from other lymphoproliferative disorders. Hence, awareness of the variations in clinical presentations and immunophenotypic aberrancies is requisite to ensure diagnostic accuracy. Materials and Methods: A retrospective study was carried out to analyze the clinical-pathological profile of patients with HCL diagnosed over a period of 81 months (2010–September 2017) in our institute. Flow cytometry was performed in all the patients, and further, BRAFV600E mutation analysis was performed by real-time polymerase chain reaction in a limited number of samples. Result: A total of 353 lymphoproliferative disorders were assessed during the period, of which 16 (4.5%) were diagnosed as HCL, which included 15 cases of classical HCL and single case of HCL-v. Striking male predominance was noted with a median age of 52 (range 22–90 years). 47% patients presented with pancytopenia, while 20% cases had leukocytosis. Three patients presented with bleeding diathesis in the form of melena and purpuric spots. The absence of splenomegaly was observed in 20% patients (4/15) while 2 (13.3%) cases had lymphadenopathy. Hypocellular marrow was observed in 13% cases. Bright expression of CD20/CD22 along with CD25/CD103/CD123/CD11c was noted in all the patients of classical HCL. Aberrant expression of CD23 and CD5 was seen in 33% ( n =5) and 6.7% ( n =1) cases respectively. CD200 was positive in all the 5/15 cases tested. The case of HCL–v presented with very high leukocyte count and exhibited a CD103/CD11c+ and CD123/CD25- profile. BRAFV600E, mutation was present in all the four patients tested who included patients with a hypocellular marrow and absent splenomegaly. Conclusion: HCL has characteristic profiles, yet it may exhibit unusual clinical and immunophenotypic presentations. Perspicacious use of flow cytometry and BRAFV600E mutation analysis will aid in the diagnosis in unprecedented cases

Background: There is paucity of information regarding the effects of cholecystectomy on serum lipid profile parameters and blood glucose levels in the patients of cholelithiasis. Therefore, this study was undertaken to compare some serum lipid parameters and blood glucose levels in patients of cholelithiasis and effect of cholecystectomy on the same.Methods: The study was conducted on 50 patients of cholelithiasis who were admitted in Surgery department, MLN Medical College over a period of one year and were planned for cholecystectomy. Complete lipid profile (which included total cholesterol, triglycerides, high density lipoproteins (HDL) cholesterol, low density lipoproteins (LDL), very low density lipoproteins (VLDL) and atherogenic index) and fasting blood glucose was evaluated pre-operatively and post-operatively on the 3rd, 7th and further after 1 month of cholecystectomy. Results: Out of 50 patients, maximum of 38% were in their 4th decade with females constituting 82% of total. There was significant elevation in fasting blood glucose and HDL cholesterol on 3rd, 7th and 30th day post-cholecystectomy as compared to pre-operative values (p value<0.0001). Triglyceride and VLDL cholesterol elevated on 3rd and 7th day but it significantly decreased one-month post-cholecystectomy as compared to pre-operative levels (p value<0.0001). However, LDL, total cholesterol and atherogenic index significantly decreased on 3rd ,7th and 30th day post-cholecystectomy.Conclusions: Cholelithiasis is an important cause associated with significant pathological changes in many lipid profile parameters, which will return towards normal after cholecystectomy. This normalization will be a time dependent phenomenon with long term beneficial effects.

Background: Preeclampsia is a multisystem disorder of unknown aetiology and recently its link with placental laterality has been explored. The objective of this study was to find the association of placental laterality with maternal and fetal outcomes in pregnancy. Study also determined the predictive ability of placenta laterality for the development of hypertension in pregnancy.Methods: A prospective observational cohort study was conducted on 200 pregnant women. Routine investigations and doppler analysis were done. Placenta position was categorized into central and lateral. Maternal and fetal outcomes were recorded. The data was entered in MS excel spreadsheet and analysis was done using statistical package for social sciences (SPSS) version 21.0. A p value of < 0.05 was considered statistically significant.Results: Study found no significant association of placental laterality with hypertension in pregnancy. Various fetal complications, birth weight, Apgar scores and NICU admission were comparable among women with central or lateral placenta (p > 0.05). Even the maternal outcomes like mode of delivery, onset of labor, indication of labor induction and caesarean deliveries were comparable among women with central or lateral placenta (p > 0.05). On applying univariate logistic regression analysis, previous history of hypertension in pregnancy was a significant risk factor for development of preeclampsia with odds ratio of 168.43 (p < 0.05).Conclusions: It can be concluded that the maternal and fetal outcomes are independent of the placenta laterality. The doppler characteristics and placenta laterality did not show any increased risk for hypertension in pregnancy. However future studies are recommended with large sample size including more women with diagnosed hypertension in the pregnancy so that a better association can be derived with placenta laterality and doppler characteristics.

Intra uterine devices (IUD) are the second most common accepted methods of contraception worldwide after sterilization. It is a long acting reversible contraceptive (LARC) which is now being promoted as a simpler, safer and effective method of contraception for those women who do not want a permanent method of contraception. Missing thread is the most common complaint seen within 6 weeks of insertion and may be the only presentation of misplaced IUD. Most women who spontaneously expel the IUD are unaware of the situation. We are presenting a case of misplaced IUD which was partially expelled and got embedded in the vagina which is a rare site of misplaced IUD.

It can act as both tracheostomy tube and stent to keep the tracheal airway open after repair of tra-cheal stenosis.Here we report a successful management of a case of 45-year old male who had Montgomery tube in situ posted for Direct Laryngoscopy assessment to rule out subglottic steno-sis before decannulation trial.

The aim of this paper is to highlight the iatrogenic aspect of secondary cleft deformity, methodologies to treat them and the importance of reporting such cases. Secondary deformities are common in cleft lip and palate patients. Primary aim of treatment incleft patients is to enhance their normal growth and development and minimize morbidity and number of operative procedures. Meticulous diagnosis, treatment planning and execution of primary surgery are of utmost importance in such cases to prevent majority of secondary deformities. Treatment of secondary deformity depends on the degree of deformity and the severity of impact on normal functions and growth. Following is a case of 16-year-old female patient who incurred secondary cleft deformities after undergoing multiple surgical interventions for congenital bilateral cleft lip alveolus and palate, last of which was conducted at the age of 6 years. Lip revision and tongue flap surgeries to close the palatal fistula were performed to address the patient‘s complaint and improve quality of life. Performing a tongue flap for anterior palatal fistula was more successful than Bardach‘s palatoplasty technique.It is important that each case encountered at various congenital defect care facilities is reported in literature to make the masses aware of probably outcomes and also help maintain a database to have more accurate data of such cases.

Introduction: Quality control of blood and its componentsensures the availability of high quality product with maximumefficacy and minimal risk to recipients. As per standardguidelines, for quality assurance of FFP, 1% of all the unitsprepared or 4 units per month are tested for stable coagulationfactors: Factor VIII and Fibrinogen levels.Material and methods: The retrospective data was collectedfrom archives of blood bank from the period of 1st January2017 to 31st December 2017 in AIMSR, Bathinda. Out oftotal 3476 units collected, 2155 were processed for componentseparation. 1.9% of Fresh Frozen plasma (41/2155) were testedfor total volume, fibrinogen content and factor VIII levels forquality control with the help of semi-automated coagulometer.Results: 97.5% of FFP samples tested had factor VIII levelsabove 0.7 IU/mL and 100% of samples had fibrinogen content>200mg/dl.Conclusion: It is concluded that quality of FFP being preparedat our blood bank meets the international standards. Regularquality evaluation and maintenance of records helps to keepup the working standards and any deficiency can be checkedand curtailed.

Aims: The objective of this in vitro study was to find out the efficacy of Novamin in remineralizing enamel surface on which artificial caries lesion had been created. The changes were analyzed using Vickers Hardness Testing Machine and Scanning electron microscope (SEM). Settings and Design: A total of 40 primary teeth were selected and divided into two groups: Control group and SHY NM (Novamin). All the samples were subjected to ph cycle protocol and assessed using Vickers Hardness Testing Machine and Scanning electron microscope. Each demineralized sample was randomly divided into two groups: Gp I –Control group, Gp II – Novamin (SHY NM). After 10-day period of ph cycle, the obtained data was analyzed statistically. Statistical Analysis Used: Pre and post groups were compared by paired t test. The significance of mean difference between the groups was done by Tukey's post hoc test after ascertaining normality by Shapiro-Wilk (W) test and homogeneity of variance by Levene's test. A two-tailed P value less than 0.05 (P < 0.05) was considered statistically significant. All analyses were performed on SPSS software (Windows version 17.0). Results: Statistical analysis showed that bioactive glass (novamin) remineralizes artificially induced carious lesion in primary teeth. Conclusions: SHY NM (Novamin) exhibited superior remineralization potential.
